Outsourcing vs. In-House: Finding the Right Operating Model


RCM leaders balance control with scale. In-house models retain institutional knowledge and direct oversight, but can strain during staffing shortages or policy shifts. Full outsourcing offers cost predictability and access to specialized talent, yet demands rigorous SLAs and governance. Co-sourcing blends both: the provider owns strategy and high-value work, while the partner handles volume-heavy tasks (e.g., follow-up, payment posting, low-complexity denials).

Selection criteria include integration with the EHR, automation maturity, security posture, and demonstrated improvement in KPIs (cash acceleration, denial reductions). Contract structures increasingly tie fees to outcomes, aligning incentives. Regardless of model, a clear RACI matrix, shared dashboards, and continuous process improvement are non-negotiable.
